The requirements change chain reaction

Change one requirement by hand and count how many other places you now have to update by hand.

The compliance checklist. The MoC table. The evidence referencing it. The document. The status. Maybe a summary somewhere. Each one manual, each one a chance to update five and forget the sixth.

That sixth one is the problem. It doesn't announce itself. It sits quietly saying "compliant" until someone looks closely — usually the authority, usually late.

Manual traceability doesn't fail loudly. It fails silently, one forgotten link at a time.
